Silverline Success Story: Suffolk Care Collaborative

The Suffolk Care Collaborative (SCC) is the Performing Provider System (PPS) for Suffolk County under the Delivery System Reform Incentive Payment (DSRIP) program. The goal of SCC is to meet the requirements of the Triple Aim Initiative – improving the patient experience of care, improving the health of populations and reducing the per capita cost of healthcare.

Challenges

Previously, Suffolk Care Collaborative managed much of their network of DSRIP providers utilizing various solutions. Information was sourced from numerous, disparate systems that contained data from multiple external sources including surveys, spreadsheets, and data from the Department of Health. This system did not provide the ability to collaborate directly with their partners to drive self-service maintenance and was lacking an effective and scalable data model to drive meaningful reports.

SCC wanted to establish a technology infrastructure that would allow them to effectively scale to manage their network of providers to meet the needs of the New York State DSRIP program.

Solution

Built on Salesforce Lightning, the modern new user experience, Silverline created a centralized Platform for key DSRIP information including Providers, Provider Groups as well as the associated administrative contact information. Through extensive automation based upon a 3-tier account hierarchy and contact relationships to Providers, Provider Groups, and locations; Silverline was able to provide new views of the DSRIP information and minimize the previously required manual collection and association processes.

Over the course of the 12-week engagement, the Silverline team assisted the SCC with the identification of their DSRIP CRM data including the interrelationships of this data. Leveraging a sophisticated data model maintaining multiple tiers within the account object, the data relationships were made available through view, reports, and dashboards. Silverline leveraged allows and process builder to allow the data to synchronize from the contacts up through the top of the account hierarchy as well as to synchronize down from the account hierarchy to the individual contacts.

Results

The business value gained by the Suffolk Care Collaborative was not only improved insights to these relationships but also the simplicity and immediate accessibility of the new information. Other benefits include:

  • Reports created in Salesforce cut down processing time from 1-2 hours to less than 30 seconds for State mandated new hire reports.
  • Consolidating spreadsheets and multiple data sources into one streamlined Platform has decreased data upload times from days to hours with the mass data upload tool.
  • With the new Lightning experience, users enjoy the modern user interface that’s optimized for speed and can work more productively.
