Deliver Holistic Care and Support Social Determinants of Health Insights
In the Spring ’19 release, Salesforce Health Cloud will give us new methods to effectively track and take action on critical social and environmental factors that can adversely affect your patients. These social determinants like homelessness, low income, and food insecurity make a huge difference in the success of your patients’ health. With the changes in Spring ’19, you can now track these determinants, take action on them through interventions, and visualize them in the interface you are already used to.
By adding the Care Determinants Lightning component to your user interface you can now quickly create barriers that affect your patient (1). Select the social determinant that is relevant to each barrier (2) and create and view interventions that address those social determinants (3).
This change introduces a handful of new standard objects including Care Barrier Type, Care Determinant Type, Care Intervention Type, Care Barrier, Care Barrier Determinant, and Care Determinant.
Streamline Patient Referrals with Referral Management
Spring ’19 also brings us new methods to refine referral processes into a more collaborative and enhanced model. Changes to the referral process will result in fewer abandoned referrals and better visibility into high importance referrals. It will now be easier to track referrals back to their referring source.
The changes in Spring ’19 support multiple referral channels and allow you to see the complete lifecycle of a referral from submission to patient. There will now be two referral records—one on the Lead and one on the Opportunity, to suit various current processes. New lead queues, new fields, new layouts all combine to deliver a refined referral functionality that can be deployed on your partner community or in your Org.
A new refined referral process wouldn’t be as much fun without new reports and dashboards to support it. You will find this and more in Spring ’19.
Complete a Health Assessment on Someone’s Behalf
No longer will you have to direct someone to a portal or other resource when you need to complete a health screening. With Spring ’19 you will be able to complete a health assessment on someone else’s behalf. Now when you are on a call you can knock that screening out right there on a single call without redirecting your patient.
Engage Patients and Members With the Salesforce Health Cloud App Template
Everyone has a phone in their hands these days, including your patients, so why not deliver a custom branded mobile app that patients and members can use? With Spring ’19 you will be able to quickly build a custom branded app using Bolt templates and Mobile Publisher that will help you connect with your patients and more. Setting and monitoring Goals, Care Teams, and Tasks from an app will help you stay connected with your patients and help them better manage their health.
Overall, Spring ’19 brings a wealth of interesting and exciting features to Salesforce Health Cloud. From referral management to social determinants there is a lot to explore.
